Eczéma des mains en 2021

L’eczéma des mains (EM) était déjà une maladie de la peau courante, même avant la pandémie de COVOD-19, avec une prévalence d’environ 10% dans la population générale. Elle a un impact socio-économique important en raison de ses coûts indirects élevés en matière de soins de santé et de son association avec les congés de maladie prolongés. L’eczéma des mains est une entité hétérogène sur le plan clinique dont la classification a toujours été controversée. Les ouvrages récents, tout en offrant des détails qui varient légèrement, partagent des similitudes dans leur approche de la classification de l’EM. Les approches permettant de classer l’eczéma des mains en certains sous-types selon la pathogénie sous-jacente sont plus courantes et font souvent la distinction entre la dermatite de contact irritante ou allergique, l’eczéma atopique des mains et la dermatite de contact protéique. Le tableau clinique est utilisé comme élément supplémentaire lorsque les facteurs étiologiques restent flous. Récemment, la question de savoir si l’eczéma hyperkératosique des mains devait être considéré comme une maladie entièrement différente et distincte de l’« eczéma des mains » en général a été soulevée. D’autres recherches ont porté sur les difficultés de diagnostic associées à l’eczéma hyperkératosique des mains. De la même manière, l’eczéma endogène dyshidrotique a été décrit comme une entité distincte et pose des problèmes liés au diagnostic et au traitement. On l’a observé en association avec la dermatite de contact allergique.
