Beyond Cold Fingers: A Practical Approach to the Management of Raynaud’s Phenomenon

Abstract

RP is far more than a benign inconvenient reaction to cold exposure or a cosmetic concern; particularly in its secondary form, it can be associated with substantial morbidity. Early distinction between primary and secondary disease is paramount, as SRP may herald serious systemic pathology that extends well beyond the digital vasculature. Effective management requires a stepwise approach guided by disease subtype, severity, and associated comorbidities. This approach begins with comprehensive lifestyle modifications, progressing through evidence-based pharmacotherapy, and incorporating procedural or surgical interventions when necessary. Given their role in early recognition of connective tissue disease and cutaneous vasculopathy, dermatologists are uniquely positioned to identify, risk-stratify, and longitudinally manage patients with RP. A comprehensive and multidisciplinary treatment strategy addressing both vascular dysfunction and systemic disease remains central to reducing morbidity and preserving long-term function and quality of life.

Author Biography

Khalad Maliyar, MD, MScCH, FRCPC

Dr. Maliyar is a dermatologist who completed his residency training in Dermatology at the University of Toronto where he served as Co-Chief Resident during his senior year. He is board certified in Canada. Dr. Maliyar also holds a Master of Public Health in Health Practitioner Teacher Education from the Dalla Lana School of Public Health at the University of Toronto. He is an Assistant Professor at the University of Toronto in the Department of Medicine, Division of Dermatology and holds a position at Sunnybrook Health Sciences Centre. His clinical interests include autoimmune diseases, immunobullous diseases, and skin lymphomas.
